All medical consent forms are interesting. The Bilateral Vasectomy consent form is certainly covering all the bases... Consider this, You sign a form that states the doctor is not responsible if you become sterile, the doctor is not responsible if you don't become sterile, the doctor is not responsible if the surgery site becomes infected and the doctor is not responsible should you die as a result of the surgery.  Well that just about covers everything.  Aren't lawyers grand...

The Head Nurse Demands my Wife Sign the Consent Form.

Some medical providers are afraid of how your wife might react, so they want a note from your wife or mother. Its not required by any medical or legal statue in the United States.  Just say NO. If that's not good enough for nurse Ratchet, change doctors.

Who Will Be In the Procedure Room During the Vasectomy?

Generally speaking in the United States, it would be you, your doctor, and perhaps your spouse if you like.  In the United Kingdom (including Australia, New Zealand, South Africa)  they seem to make much more out of this procedure, so you can expect a nurse and even a anesthesiologist to assist as well (I've read recent literature that general anesthesia is used routinely!!!).  I have heard tales of doctors and or nurses in training in attendance as well.  If it's a concern, ask during your consultation who will be attending your party.

I have been overwhelmed with email requesting more information.  The research on the FAQ page only added 4 hours to the site, so the information was included.  I added the procedure slide page after being bombarded with request for more information.  It also happens to be one of the most popular pages on the site.

What Equipment and Software Did You Use To Make This Web Page?

All images on this site have been recorded with a Sony® TRV-900 3 ccd digital camcorder (DV) set to progressive scan, and a Professional Fuji® medium format GS645zI still camera, using Fuji professional NHGII 800 film.

Computer hardware is a Dell® Precision 610 workstation with a 500-MHz Intel III Xeon processor running high performance SCSI hard disk drives with Adaptec® Raidport data stripping installed.  Software is Microsoft® FrontPage 2000.  The site special audio effects can only be heard using Microsoft's® Internet Explorer 5.0.
